The Council's learning disability (LD) team is a multi agency team, which works with adults with a diagnosed learning disability and their families to access the right supports to alleviate risk, meet needs, achieve fulfilling lives and outcomes.
The LD team is a multi-agency team made up of social workers, senior resource workers and community learning disability nurses. We work closely with a number of other services to support adults with learning disabilities to have the appropriate tools and supports to live safely in the community promoting least restrictive practice.
To access the service, you must:
We work closely within a wider multi-disciplinary team which includes: occupational therapists, speech and language therapists, psychiatrists, psychologists, dietician, advocacy, physiotherapists and the Whins day service provision.
Please note, our health colleagues' criteria is 18+ years.
The service provides day opportunities and encourages people to choose activities designed to achieve their individual outcomes, enabling them to remain a part of your community and be as independent of services as possible.
Our staff are trained and knowledgeable, and able to provide the right support for to meet an individual’s needs.
The centre base is fully accessible, with equipment such as tracking hoists, plinths, accessible bathrooms and showers.
We offer a variety of activities both within the Whins Resource Centre, and in the local community which aim to help individuals meet their outcomes, including learning, staying healthy, maintaining ability and social opportunities. Activities include creative expression such as art, drama, dance and music, health and wellbeing such as playing sport or exercising and attending classes or events in the local area, and learning how to travel independently.
The principles and values reflected in the Health and Social Care Standards: My support, my life are upheld by the service.
The service works closely with other social work and health colleagues including physiotherapists, occupational therapists, dietician, district nursing teams, and speech & language therapists to help individuals achieve their outcomes.
Accessibility: You will need a social work assessment to access the service. Other referrals can be made either through GPs or through the NHS Forth Valley REACH team, in consultation with the Social Work service.
Cost: This will be subject to financial assessment.
An adult with a learning disability can be referred to our team through another professional (eg their GP, advocacy worker), their family or they can refer themselves by contacting our duty service on the number below. Our duty worker will then gather as much information as possible about your circumstances, supports, strengths, unmet needs, risks and challenges. If the worker determines that you would benefit from further Social Work assessment, they will add your name to the waiting list, or they may signpost you to another service.
Upon reaching the top of the waiting list, a worker will be assigned to you. They will visit you at home and meet with you and any other relevant parties to understand more about your circumstances, goals and hopes. They will then write a report making recommendations about what support you may require to keep you safe. This may include:
