Staying Independent at Home

Overview

Clackmannanshire and Stirling Health and Social Care Partnership (HSCP) is committed to improving how we deliver social care, ensuring it is personalised, responsive, and sustainable. This approach, called Right Care, Right Time, focuses on ensuring you receive the right level of support when you need it consistently across the HSCP. 

Eligibility

People in the following community care groups may receive home care services:

  • Older people and those with dementia
  • People who have physical disabilities and/or sensory impairments
  • People who have mental health problems
  • People who have learning disabilities

Some people may also be eligible for Self Directed Support (direct payments) to purchase their own service; the assessment and care management team can provide information about this.

If formal support is not required, we can still provide advice and signpost you to community groups, family/friends, or other supports that may meet your needs. If your circumstances change and additional support is needed, you may request an assessment at any time.

Service specifics

The HSCP has a duty to review care and support regularly to ensure it continues to meet agreed needs and outcomes. Where appropriate, support is funded through a personal budget based on assessed needs and aligned to the Social Care (Self-Directed Support) (Scotland) Act 2013.

Process

The HSCP has a duty to regularly review your care and support to ensure it continues to meet your needs and agreed outcomes. A worker from social work services will contact you to carry out these reviews. 

When we assess your needs or review your current support, we follow an asset-based approach. This means we first look at the support already available to you from your community, family, or other existing resources. If gaps remain after exploring these options, we will consider additional paid support. 

If paid support is needed, we will calculate a budget based on your assessed needs and personal outcomes. This budget aligns with the Social Care (Self-Directed Support) (Scotland) Act 2013, which ensures the budget reflects the reasonable cost of meeting your needs. 

Where support services are required, for example care at home, support providers, or personal assistants, costs are guided by the HSCP’s schedule of rates and reviewed annually. Your budget and support plan will be reviewed and adjusted as your needs change over time. The budget is based on an equivalency model, which means that an individual with eligible needs will be entitled to a personal budget, equivalent to the cost of arranging traditional services to meet that person’s eligible needs.   

The assessment and care management team will identify the most appropriate service for your needs and this may include access to our reablement service who will work with you to improve your daily living skills.

Depending on a person's financial circumstances there may be a charge for the service, although help with personal care is generally provided free of charge to older people, ie those 65 and over.
