Hospital Discharge Service overview

What does the team do? 

Provides support for people currently in hospital who need help to return home safely. 

The team assesses care needs to support discharge from hospital and arranges care packages, support plans or long-term placements.

Reablement Support Service overview

What does the team do?  

Provides short-term support at home to help people regain independence. 

The team carry out assessments of need in people’s own homes following discharge from hospital or in response to a crisis or change in needs in the community.
