Mobility, Adaptations and Equipment

Overview

This service supports adults in Clackmannanshire who are experiencing difficulty with everyday tasks due to illness, disability, frailty, injury, or reduced mobility.

It provides equipment, advice, and home adaptations to help people remain safe, independent, and confident in their own home for as long as possible.

Support may include:

• Daily living equipment (bathing, toileting, kitchen and household aids)
• Mobility support equipment
• Sensory support equipment
• Telecare/technology enabled care
• Minor home adaptations
• Major home adaptations
• Hospital discharge equipment support where required

The service also aims to:

• Prevent avoidable deterioration in independence
• Reduce risk of hospital admission where appropriate
• Support timely discharge from hospital
• Reduce pressure on carers and support safer caring arrangements
• Promote independence and wellbeing rather than only responding to crisis situations

Eligibility

This service is for adults living in Clackmannanshire who have care, mobility, or functional difficulties due to:

• Disability
• Long-term health condition
• Frailty
• Injury or recovery needs
• Reduced independence at home

Service specifics

Hospital Discharge and Reablement Support

If you are leaving hospital, an Occupational Therapist or Physiotherapist may assess your immediate needs and arrange equipment and/or a package of care to support a safe discharge into the community. 

Please note that hospital staff are generally able to support with immediate discharge equipment needs, but they are not able to directly progress minor or major adaptation works within the community. 

Where ongoing adaptations are required, a referral will need to be submitted on your behalf to Adult Social Care for further assessment and consideration.
