This service supports adults in Clackmannanshire who are experiencing difficulty with everyday tasks due to illness, disability, frailty, injury, or reduced mobility.
It provides equipment, advice, and home adaptations to help people remain safe, independent, and confident in their own home for as long as possible.

If you are leaving hospital, an Occupational Therapist or Physiotherapist may assess your immediate needs and arrange equipment and/or a package of care to support a safe discharge into the community.
Please note that hospital staff are generally able to support with immediate discharge equipment needs, but they are not able to directly progress minor or major adaptation works within the community.
Where ongoing adaptations are required, a referral will need to be submitted on your behalf to Adult Social Care for further assessment and consideration.
