Support for Care Leavers

Overview

If you’re moving on from care, or have already left care, you don’t have to manage everything on your own. 

The throughcare and aftercare (TCAC) team is here to support you as you move into adulthood. We work alongside you to help with things like housing, money, education, work, health, and staying connected to people who matter to you. 

From age 19 onwards, support from TCAC is voluntary, which means 

  • you don’t have to accept help if you don’t want to 
  • you can come back for support later, even if you said no before 

You have the right to help, and we will respect your choices, voice and experiences. 

Eligibility

You may be able to get support from TCAC if

  • you were looked after by the local authority on or after your 16th birthday 
  • you are aged 16–25 
  • you are leaving care or have already left care 

Being looked after can include

  • foster care 
  • residential care 
  • looked‑after kinship care 
  • being on a Compulsory Supervision Order

Service specifics

What support can look like

Support looks different for everyone. We start by asking what matters to you. 

This might include help with 

  • finding or keeping a place to live 
  • money, benefits, grants or budgeting 
  • education, training, college or university 
  • work, apprenticeships or volunteering 
  • physical or mental health 
  • building independence skills 
  • keeping in touch with people who are important to you 

We’ll agree support with you, at your pace. 

Process

  • Get in touch 
    • You can contact us yourself or ask someone you trust to help you make contact. 
  • If you already have a social worker 
    • Your social worker will support you to link in with Throughcare and Aftercare Team and can help make sure the right support is in place for you. 
  • Have a conversation 
    • We’ll arrange a chat at a time and place that suits you to talk about what’s going well and what support might help. 
  • Agree support together 
    • If you want ongoing help, we’ll agree a plan with you, based on what matters to you and at a pace that feels right. 
  • You stay in control 
    • You can change your mind, pause support, or come back to us later - the door stays open. 

We work closely with other services across Clackmannanshire so you can get the right support at the right time. 

Sometimes this might mean we suggest a referral to another service that can better meet a specific need. We will always talk this through with you first, explain why, and listen to your views before anything happens. 

Request for Care Leavers form

If you left care at age 16 or over, you can fill out this Request for Care Leavers form to request support from us at any point between the ages of 16 and 26.
