Overview

The reablement team is a short-term care at home assessment service which provides tailored care and support to people in their own homes.

The team are part of Adult Social Care in the Health and Social Care Partnership (HSCP), and consists of a team leader, senior care officers, operational supervisors, resource support officers and reablement support workers.

The team carry out assessments of need in people’s own homes following discharge from hospital or in response to a crisis or change in needs in the community.

The aim is to maximise people’s recovery in their own familiar environment. The service will arrange an initial package of care and staff will work with the service users and their family to develop a support plan with specific goals to increase confidence and skills. If an ongoing package of care is required at the end of the reablement period, this will be arranged by the team who will continue to provide care until this is sourced.    

The team operates 7 days a week from 7am until 9.10pm.  

Eligibility

The team offer a service to those adults who meet the critical and substantial level of the social work eligibility criteria. Eligibility is confirmed upon receipt of a referral when initial screening will be carried out. There can be a waiting list, and people are prioritised on need.  

Signposting

As part of the screening and/or assessment process the team will signpost to other agencies including third sector and/or statutory services where appropriate.

Service specifics

The team provide a service to those people who are in their own homes and require an assessment and care to support their discharge. The service is mostly for older adults but can provide care to younger adults who meet the criteria.  

Process

Referrals are usually received by submission of a Single Shared Assessment (SSA) or Request for Assistance (RFA) or by a phone call to Social Services.

Referrals are received and screened to ensure they meet the eligibility criteria. The service will check if a suitable assessment/package of care is available and liaise with the hospital or service user if they are at home, to arrange for this to begin.

The timescale for assessment and support is dependent upon level of risk, and demand versus capacity at the time.
