Hospital Discharge Service overview

Overview

The hospital discharge team are part of Adult Care, Health and Social Care Partnership (HSCP).

The team consists of care managers (social workers, occupational therapist and nurse), as well as social care officers. The team carry out assessments of need to facilitate discharges from any hospital within the Forth Valley area, or any intermediate care setting. This may be for packages of care, support plans or long-term care placements. The team also provides a duty system for hospital cases. 

The team operate Monday-Friday, 9am-5pm.

Outwith these times, any urgent referrals/calls will be directed to EDT. The team work a hybrid model and have bases in Kilncraigs, Forth Valley Royal Hospital (FVRH) and Clackmannanshire Community Healthcare Centre (CCHC).   

Eligibility

The team offer a service to those adults who meet the critical and substantial level of the eligibility criteria. Eligibility is confirmed upon receipt of a referral when initial screening is carried out. 

Signposting

As part of the screening and/or assessment process the team will signpost to other agencies including third sector and/or statutory services where appropriate. 

Service specifics

The team provide a service to those people who are in a bed-based setting and require an assessment to support their discharge. It does not provide a service to children (under 16yrs, or adults with a diagnosed mental illness or learning disability up to age 65yrs).

Process

Referrals are usually received by submission of a Single Shared Assessment (SSA) or Request for Assistance (RFA). Occasionally people are referred via a Public Protection Referral or Vulnerable Persons Report (VPD).

Once a referral is received and screened to ensure they meet the eligibility criteria, the case will be allocated to an appropriate worker.

Timescale for allocation is dependent upon level of risk, and demand versus capacity.
