Assessment and Care Management Service

This service has 6 teams, comprising:

• Intake/Duty
• Locality
• Learning Disability
• Mental Health
• Hospital
• Intermediate Care

The teams consist of:

• Occupational Therapists
• Social Workers
• Social Care Officers
• Mental Health Officers

The teams can provide an assessment of your community care needs and any problems you may be facing. The outcome of which may identify services to help you, such as:

• Community Alarm Service/Telecare
• Equipment and Adaptations for People with a Disability
• Respite Care
• Day Care
• Residential Care
• Nursing Home Care

Domestic help will only be provided when there is exceptional need. Details of local care providers can be given on request.
