Palliative and end of life care helps improve the quality of life for someone who has a life limiting illness, by offering services, advice, information, referrals and support. Whilst this can be a challenging time, there are important conversations involved.
We need you to provide views from both personal and professional perspectives to help us develop our strategic thinking around community palliative and end of life care services.
