Clackmannanshire Community Health Partnership
A Clackmannanshire Community Health Partnership (CHP) was established in 2005 to provide comprehensive local health care and community services in partnership with the Council and local organisations.
Central government guidance for modernising the health service is set out in Scotland's Health White Paper, ‘Partnership for Care’(February 2003).
The Community Health Partnership is part of the new structure of local health and community care services. It will build on the achievements of the Local Health Care Cooperatives and is intended to make measurable improvements in the health of the local population by providing higher quality, accessible, joined-up services to local communities.
The key aims of the Partnership are:
- To ensure that patients and their carers, local health and social care agencies and the full range of healthcare professionals are involved.
- To establish a substantive partnership with the local authority.
- To have greater responsibility and influence on how the Health Board’s resources are deployed.
- To play a central role in service redesign locally.
- Focus on integrating primary and specialist health services, at local level.
- To deliver health improvement for their local communities.
The Community Health Partnership includes NHS Forth Valley Primary Care and Community Health Services, Clackmannanshire Council, voluntary organisations and representation from members of the public. A Public Participation Forum has also been set up to represent local views.
Clackmannanshire will also have a new Community Hospital in Sauchie in 2008 and plans are underway to develop this local resource which will provide health clinics and out-patient services, in-patient beds, community care and day services.
