Community Care Plan 2005-2008

The Clackmannanshire Community Care Plan 2005-2008 sets out the key priorities for the Council, in partnership with NHS Forth Valley, for the provision of support and services to people who have community care needs in Clackmannanshire.

The Plan set out a framework for services and an update on progress from the 2002-2005 Plan. It addresses

  • the needs of older people;
  • people with dementia;
  • people who have mental health problems;
  • people who have physical disabilities;
  • people who have sensory impairments;
  • people who have learning disabilities;
  • people who need social support or services for other reasons, eg illness, problems arising from substance misuse, or homelessness.

The Plan also takes into account the needs of Carers.

Contained within the Plan are reference points and links to more detailed health, housing and social services plans which are in place. This is a time of considerable change and development in the way services are provided in partnership with other organisations, examples of which are:

The Community Care Plan will be updated for 2008-2011 to reflect the changes that are taking place.
